One way is to select DVD with DVD case in top right selection panel and then with the Case pane selected go to the middle bottom selection panel and select Plain Paper Slim Case (A4,Legal or US) (which is about halfway down).
See if this fits your need!

Hi,
Click on the CD / DVD setting in the upper right corner of the programs screen and choose "DVD with DVD Case." Click on the Case tab and then chose one of the "Plain Paper Slim DVD Case" templates.
